  • After you complete the form, close the window/tab and return to this page to complete Step 2.
  • To mail a hard copy of the registration form, click the “print” link above.

  • You will be redirected to another page to pay. There will be a $5 online processing fee (which will appear as a “shipping” charge).

Partner fees include two luncheons, two evening meals, a film screening, and any other special events.

Onsite payment of the registration fee is $350.

Cancellation Policy: If cancellation of your registration becomes necessary, you may contact Dr. Gwen Westerman at the above address, email at or fax at (507) 389-5362. Cancellations must be made by February 15, 2019. A $10 administrative fee will be assessed on all cancellations. Confirmed registrants who do not attend the conference or who cancel after February 15, 2019 are responsible for the entire registration fee.
